Our client is seeking a compassionate and experienced Registered Nurse to join their extraordinary Home Health care team. This full-time position involves providing management and delivery of patient care plans in Carnegie/Crafton, PA and surrounding areas.
The role requires on-call availability on weekends and local travel to patient locations. The successful candidate will have an associate degree, an RN license in the state of work, and at least one year of RN experience.
PAY AND BENEFITS:
Competitive compensation, including base pay and bonus
$15K Sign-On Bonus (paid out quarterly over the course a year)
Outcome based Bonus ($1K per quarter)

SCHEDULE AND SHIFT DETAILS:
On-call Friday, Saturday, and Sunday from 5:00pm to 10:00pm + 8 unit

THE IMPACT YOU WILL HAVE IN THIS ROLE:
The Registered Nurse assesses human responses and plans, implements and evaluates nursing care for individuals or families for whom the nurse is responsible. The Registered Nurse is fully responsible for all actions as a licensed nurse and is accountable to patients for the quality of care delivered.
YOUR KEY RESPONSIBILITIES:
Lead and collaborate to provide management and delivery of patient care plans
Support patients in Carnegie/Crafton, PA and surrounding areas
Spend ~20% of the day driving to/from patient locations
Consistently meet critical deadlines for charting
Self-manage time and schedule
YOUR PRIMARY RESPONSIBILITIES:
Assures safe delivery of nursing services in alternative care settings.
Active participation in on-going interdisciplinary communication specific to each patient with review of patient status and plan of care.
Directing and organizing ancillary care as appropriate.
Adherence to policies and procedures regarding medical record documentation and Performance Improvement activity.
Home Infusion visits to administer Infusion Therapy in the home setting.
Implements current nursing practices.
Patients clinical assessments are completed as appropriate and communicated to other team members.
Nursing interventions are implemented to meet patients needs and in accordance with physician orders.
Physician orders are accurately received, implemented, and evaluated for patient outcome in a timely manner.
Patients are appropriately referred to other health care support systems or community resources based on identified needs.
Referral information is documented accurately in a timely manner and shared with other team members.

ESSENTIAL QUALIFICATIONS:
Associate degree or higher
1+ years of experience in ANY of the following:
Clinical Documentation
Clinical Informatics
Clinical Quality
1+ years of experience in ANY of the following:
Electronic Medical Record (EMR)
Other EMR
Practice Partner EMR
Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
ALL of the following valid licenses/certifications:
American Red Cross Advanced Life Support (ARC ALS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
