Home Health Speech Language Pathologist (PRN)

Lakeshore Area | Flexible Scheduling

What You’ll Do:

  • Evaluate patients and develop individualized speech therapy plans of care

  • Perform initial and ongoing assessments in accordance with OASIS guidelines

  • Provide speech-language therapy services in the home setting

  • Educate patients and caregivers on communication, swallowing, and cognitive strategies

  • Collaborate with physicians and interdisciplinary team members

  • Adjust care plans and request physician orders as needed

  • Ensure timely, accurate clinical documentation

What You'll Need:

  • Master’s degree from an accredited educational program in Language Pathology

  • Current, unrestricted SLP license in the state of work  

  • Minimum one (1) year of experience as a SLP in a clinical care setting

  • Able and willing to travel within branch/office coverage area.

  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.

  • Must be able and willing to travel 50%
