The responsibilities of our Wound Care Nurses include: 

• Perform comprehensive wound assessments, including measurement, staging, and documentation of wound characteristics


• Develop and implement individualized wound care plans in collaboration with the physician and interdisciplinary team


• Administer prescribed treatments and procedures, including wound debridement (as applicable by license and certification), dressing changes, and infection management 


• Educate patients and caregivers on wound care management, nutrition, skin care, and prevention strategies


• Monitor and document wound progression, response to treatment, and any signs of complications


• Communicate effectively with the physician and care team to ensure continuity of care and timely updates on patient condition


• Maintain accurate and timely clinical documentation in accordance with agency and regulatory requirements


• Participate in quality assurance initiatives and wound care performance improvement activities


• Ensure compliance with infection control standards and universal precautions at all times


• Serve as a clinical resource for staff nurses regarding wound management best practices

• All other duties as assigned 

Qualifications: 

• Graduate from an accredited School of Nursing 

• Licensed in the state as an RN or LPN


• Certification in Wound Care (WCC, CWON, CWCN, or equivalent)

• Minimum of 1-2 years of clinical nursing experience; home health or wound care experience preferred


• Strong knowledge of wound healing principles, wound dressings, and negative pressure therapy (VAC)

• Knowledge of Medicare and Medicaid guidelines 

• Criminal background check 

• Must have a current CPR certification

Job Conditions: 

• Valid driver's license required 

• Ability to access patients' homes, which may not be wheelchair accessible. 

• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care 

• Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
