What You’ll Do:

• Provide compassionate, hands-on patient care, including end-of-life support


• Follow individualized care plans and keep documentation accurate and timely


• Educate patients and families so they feel supported and informed


• Work closely with an interdisciplinary team to deliver the best possible care


• Manage pain and symptoms to ensure comfort and dignity


• Be part of a team that truly supports each other and the patients we serve

What We’re Looking For:

• Graduate of an accredited nursing program


• At least 1 year of nursing experience (hospice, home health, or med/surg is a plus)


• Active LPN license
  


• Strong communication and organizational skills


• Valid driver’s license and reliable transportation

Schedule: Full-time | Day shift | Rotational on-call
