Overview

A growing homecare organization is seeking a sharp, organized, and proactive Homecare Coordinator to manage patient cases, coordinate caregiver schedules, and ensure smooth day-to-day service delivery.

This is a fast-paced role for someone who can juggle multiple moving pieces, solve problems quickly, and make sure patients receive the care they need without interruptions.

The right person is a strong communicator, detail-oriented, and comfortable taking ownership of cases from start to finish.

Key Responsibilities

• Coordinate caregiver assignments and schedules based on patient needs and availability

• Manage patient cases and ensure consistent staffing coverage

• Fill open shifts, handle callouts, and resolve scheduling conflicts

• Communicate regularly with patients, families, and caregivers

• Monitor active cases and follow up on outstanding staffing needs

• Handle day-to-day issues and escalate more complex situations when necessary

• Work closely with recruitment to identify caregiver needs and fill open cases

• Maintain accurate scheduling records, case updates, and documentation

• Coordinate with HR, compliance, and other departments to ensure caregivers are cleared and ready for assignments

• Ensure timely follow-through and minimize interruptions in patient care

Ideal Candidate

• Experience in homecare coordination or scheduling strongly preferred

• Highly organized and able to manage multiple cases and priorities

• Strong communication and interpersonal skills

• Quick thinker who can solve problems under pressure

• Detail-oriented with excellent follow-through

• Comfortable working in a fast-paced, high-volume environment

• Proactive, dependable, and able to work independently

• Familiarity with LHCSA operations and homecare scheduling systems is a plus
