We are looking for a compassionate, organized, and dependable Hospice Aide Scheduler to join our hospice team. This position plays an important role in ensuring our patients receive consistent, timely, and compassionate care by coordinating hospice aide visits and maintaining schedules that meet both patient needs and staff availability.
The ideal candidate is someone who thrives in a fast-paced environment, communicates well with patients, families, caregivers, and staff, and can quickly adjust when schedules change. Experience in hospice, home health, healthcare scheduling, or staffing is preferred.
Coordinate and maintain daily and weekly schedules for Hospice Aides/CNAs based on patient needs and care plans.
Ensure patients receive their scheduled hospice aide visits consistently and appropriately.
Match Hospice Aides with patients based on location, availability, skills, and patient needs.
Communicate schedule changes with Hospice Aides, nurses, patients, families, and other members of the interdisciplinary team.
Monitor missed, declined, canceled, or rescheduled visits and work quickly to address coverage needs.
Assist with filling open shifts and finding appropriate coverage when an aide is unavailable.
Maintain accurate scheduling records and documentation.
Review upcoming schedules to identify potential staffing gaps before they become an issue.
Work closely with the clinical team to understand changes in patient needs and adjust aide schedules accordingly.
Help ensure schedules remain consistent with patient care plans and organizational policies.
Respond professionally and compassionately to questions or concerns from patients, families, and staff.
Assist with onboarding and scheduling new Hospice Aides as needed.
Maintain confidentiality and protect patient information at all times.
Support the hospice team with other administrative and scheduling responsibilities as needed.
Excellent organizational and time-management skills.
Strong attention to detail.
Ability to multitask and prioritize in a fast-paced environment.
Excellent communication and customer-service skills.
Ability to remain calm and solution-focused when unexpected scheduling issues arise.
Comfortable communicating with patients, families, caregivers, and healthcare professionals.
Ability to work independently while also being a strong team player.
Basic computer skills and the ability to learn scheduling and electronic medical record systems.
Previous experience in hospice, home health, healthcare scheduling, staffing, or a similar setting is preferred.
Knowledge of hospice care and the role of a Hospice Aide/CNA is a plus.
The right person understands that scheduling in hospice is more than filling a calendar. It is about making sure patients have the people and support they need during an incredibly important time in their lives.
