We’re hiring a Hospice Case Manager (RN) who wants meaningful patient relationships, weekday stability, and the autonomy to provide compassionate care where patients are most comfortable: their homes.

You’ll step into a field-based hospice role focused on assessments, symptom management, care planning, patient and family education, and interdisciplinary coordination. You’ll have independence in the field while remaining connected to a supportive hospice team.

What makes you a great fit
• You hold an active Georgia RN license
• You maintain current CPR certification
• You have recent nursing experience, ideally in home care or hospice
• You’re comfortable assessing patients and managing symptoms independently
• You communicate effectively with patients, families, physicians, and your care team
• You have a valid driver’s license and reliable, insured transportation

Compensation & Benefits
• Base salary starting around $92,000, with consideration for experience
• Mileage reimbursement
• $60 monthly cell phone stipend
• Medical insurance with 75% employer-paid coverage
