Hospice Coordinator

A facility near La Crosse, WI is seeking a Hospice Coordinator to join their hospice and palliative care team!

Starting Pay: $44 an hour and increases with experience

Hospice Coordinator Opportunity:

• Full-time, permanent position
  


• Schedule: Day Shift; Mon-Fri
  


• Signing Bonus available!
  


• Hospice Coordinator oversees all aspects of hospice patient care, collaborating with the interdisciplinary team to develop, implement, evaluate, and adjust individualized care plans while ensuring quality symptom management, regulatory compliance, and continuity of care across settings.
  


• Provides clinical leadership and supervision for hospice staff and volunteers, including staffing, training, performance management, quality assurance, admissions coordination, patient/family education, and maintaining positive relationships with patients, families, physicians, facilities, and community partners.
  

Hospice Coordinator Qualifications:

• Hospice Coordinator must have RN license with the privilege to practice in the state of Wisconsin
  


• 2 or more years of clinical nursing experience required
  


• Experience in Hospice preferred
  


• Experience in management preferred
