JOB REQUIREMENTS
• Licenses: Active Indiana License or IMLC
• Board: Board Certified or Board Eligible
• Certifications: BLS Required
• Flu and COVID Vaccination Required or Approved Exemption
• Case Logs Required
DUTIES & RESPONSIBILITIES
• Provide Inpatient Hospice Coverage
• Manage Hospice Admissions and Follow -Up Care
• Participate in Night and Weekend Call Rotation
• Complete Documentation and Order Entry in Epic
• Coordinate Care with the Hospice Team
