Job Description - Hospice RN

Job Description

 

  • Coordinate the patient/family care and services including those of the IDT. (interdisciplinary team)
  • Appropriately utilizes the skills and resources of contract personnel such as physical therapy when appropriate.
  • Develops and updates plan of care with the IDT and others involved in the patients care.
  • Provides skilled nursing as determined by the plan of care.
  • Educates patient/family and other primary caregivers, in appropriate care techniques.
  • Serves as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers, and others as well as .
  • May participate in providing bereavement support to families and associated bereavement activities.
  • Ensure and document appropriate use of medication, durable medical equipment and supplies
  • Participates in team meeting.
  • Provide appropriate, coordinated care plans.
  • Monitors the recertification/reauthorization process
  • Reports necessary information to the Team Leader and other members of the IDT for on-call shift report.
  • Responds to on-call issues as clinically appropriate
  • Performs on-site evaluations and supervision of Grace HHAs.

 

 

Qualifications

 

  • Currently licensed to practice nursing in the state
  • A minimum of two years nursing experience in an acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience.
  • Communication Skills
