Hospice Social Worker

Salary: $60k-70k/yr (BOE) 

Schedule: M-F| 8:00AM-5:00PM +(on-call rotation) 

Location: Toledo, Ohio and surrounding area

Requirements of the Hospice SW role:

• 
  

  Licensed Social Worker (LSW) in the state of practice. 

  
  


• 
  

  Bachelor's Degree from a school of Social Work accredited by the Council of Social Work Education, Master's degree preferred. 

  
  


• 
  

  One year of social work experience in a healthcare setting. 

  
  


• 
  

  Current Driver's License, vehicle insurance, and access to dependable transportation.

  
  

About the Hospice SW role:

• 
  

  Provide medical social services to the patient and/or family according to the Plan of Care.

  
  


• 
  

  Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient's care.  

  
  


• 
  

  Participate in IDG meetings and activities and the development of the patient plan of care. 

  
  


• 
  

  Observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and response to treatment.

  
  


• 
  

  Maintain and submit written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes.

  
  


• 
  

  Confirm weekly, the scheduling of visits with the patient's case managers to facilitate coordination of other staff visits. 

  
  


• 
  

  Assess the patient's and family member's ability to cope with the patient's passing. 

  
  


• 
  

  Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems.

  
  


• 
  

  All other duties as assigned
