Hospital Access Navigator (RN)

Hospice of Marion County, a member of Empath Health is seeking a Weekend Hospital - Access Navigator to join our Admissions team. This full-time, leadership role offers an opportunity to make a meaningful difference in patient access and care coordination across our hospital partners.

Friday - Sunday 8am-8pm | 12 hour shifts, 36 hours a week

What you'll Do

• Provide daily leadership, oversight, and support to the Access Team staff within the Admission–Care Navigation Department.


• Serve as the primary liaison to hospitals, communicating Empath Health’s Mission, Vision, and values to partners and stakeholders.


• Ensure compliance with all state and federal regulations, as well as standards of care established by accrediting agencies.


• Coordinate and evaluate patient access and care navigation processes to promote quality, efficiency, and continuity of care.


• Monitor performance and support staff development through coaching, training, and performance feedback.

What You'll Need

• Education and/or Experience:  A.S. degree required -  Bachelor’s degree (B.A.) from four-year college or university and Master’s degree preferred.


• Two years Hospice experience with at least two years of management experience preferred.


• At least five years total years experience as an RN preferred.


• Licensure:  RN


• Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.  Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file. This position requires travel between multiple locations.
