Job Description - Housing Retention Case Manager


The Housing Retention Case Manager (HRCM) builds meaningful relationships with individuals who have recently obtained housing and supports them in maintaining housing stability as they reintegrate into the community. This role provides participant-centered case management that is responsive to each individual's pace, goals, and identified needs.

The HRCM works primarily in the community and by phone, meeting participants where they are most comfortable. Services may include housing stabilization, benefits navigation, employment support, utility assistance, resource coordination, advocacy, and referrals that promote long-term housing success.

The HRCM reports directly to the Housing Manager and works collaboratively with the Housing Team, Program Manager, and community partners to ensure participants receive comprehensive, coordinated support. Work schedules are generally Monday through Friday, with flexibility required to accommodate participant needs, community events, and agency operations.


Essential Duties and Responsibilities

Responsibilities include, but are not limited to:

  • Build trusting relationships with participants using trauma-informed, person-centered, harm reduction principles.
  • Provide ongoing housing retention services that support participants in maintaining safe, stable, long-term housing.
  • Meet with participants in the community, in their homes, by phone, or at other mutually agreed-upon locations.
  • Conduct regular participant visits, including bi-monthly home visits or more frequently as needed.
  • Complete Coordinated Housing Access (CHA) assessments and maintain HMIS documentation as applicable.
  • Complete Releases of Information (ROI) and coordinate care with Continuum of Care partners and other service providers.
  • Assist participants with housing navigation, lease compliance, housing-related problem solving, and communication with landlords or property management.
  • Coordinate with the Housing Authority of Clackamas County (HACC) and other housing providers to support successful housing placement and retention.
  • Assist participants with utility assistance, budgeting, employment searches, benefits applications, and other supports that contribute to housing stability.
  • Provide referrals and advocacy for community resources, including healthcare, behavioral health, food assistance, SNAP, SSI/SSDI, public assistance, employment services, transportation, and long-term case management.
  • Advocate for participants with landlords, healthcare providers, government agencies, and community partners to reduce barriers to housing stability.
  • Serve as the primary point of contact for assigned participants and maintain an active caseload.
  • Develop and maintain strong working relationships with participants, staff, volunteers, landlords, and community partners.
  • Participate in regular supervision, staff meetings, training, and professional development.
  • Complete other duties as assigned by the Lead and or Housing Manager.

Requirements

Qualifications

Preferred qualifications include:

  • Experience in case management, housing services, social services, behavioral health, or related human services.
  • Knowledge of Housing First, harm reduction, trauma-informed care, and person-centered service delivery.
  • Experience navigating community resources, public benefits, and housing systems.
  • Ability to build trusting relationships with individuals experiencing homelessness or housing instability.
  • Strong communication, organization, documentation, and problem-solving skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Valid driver's license, reliable transportation, and ability to travel throughout the community as needed.

Physical Requirements

  • Ability to frequently walk, stand, bend, and travel throughout the community.
  • Ability to lift and carry up to 50 lbs
  • Ability to safely enter participant homes, apartment buildings, shelters, and outdoor environments.

Work Schedule

This position is generally scheduled Monday through Friday during regular business hours. Flexibility is required to meet participant needs, attend community events, conduct field visits, and respond to housing-related situations as necessary. A significant portion of work is performed in the field, including participant home visits and meetings with community partners.
