Job Description - Housing Stabilizer - Thriving Families

Organizational Overview



Upward Bound House (UBH) is dedicated to ending the cycle of intergenerational family homelessness. We compassionately provide a continuum of services ranging from crisis housing to permanent housing and homelessness prevention. Summary


The Housing Stabilizer supports families (ages 18–30) in maintaining long-term housing stability after exiting homelessness. This role focuses on barrier reduction, housing retention, life skills support, and landlord/tenant relationship strengthening.


Participants are often dual-enrolled in UBH’s Rapid Rehousing (RRH) programs, so the Housing Stabilizer works closely with RRH Case Managers, Employment Specialists, and Mental Health Clinicians to provide coordinated, non-duplicated services.


This program is supported by private funders and requires strong documentation, home visiting, and care coordination.



ESSENTIAL FUNCTIONS


Housing Stability & Support


· Conduct home visits and weekly follow-ups to support participants with maintaining housing.


· Provide stabilization-focused services (not clinical case management): coaching, problem-solving, budgeting support, childcare navigation, and resource linkage.


· Develop and update Housing Stabilization Plans with participants.



Landlord & Tenancy Support


· Serve as a liaison between participants and landlords to address concerns early.


· Troubleshoot habitability issues, neighbor conflicts, rent challenges, and other tenancy risks.


· Educate landlords about UBH support, communication protocols, and available resources.


Cross-Program Collaboration


· Participate in case conferences with RRH, Mental Health, and Employment teams for dual-enrolled families.


· Coordinate services and ensure goals and interventions are aligned across programs.


Data, Documentation & Assessments


· Maintain accurate records of home visits, progress notes, and landlord interactions.


· Complete funder-required assessments, including the Family Resources Scale every six months.


· Support reporting for private donor requirements.


Resource Support & Economic Mobility


· Connect participants to childcare, healthcare, transportation, community resources, and financial supports.


· Partner with Employment/Economic Empowerment staff to support income growth and work readiness.



QUALIFICATIONS


· BA in Social Work, Human Services, or related field or equivalent experience.


· Experience working with TAY (18–30), families, or formerly homeless populations.


· Ability to conduct home visits and provide field-based services.


· Strong communication, organization, and problem-solving skills.



MANDATORY REQUIREMENTS

4) Successful completion of background screening.


5) CPR/First Aid training


6) Must have Valid CA Driver's License


7) Must provide proof of insurance coverage


8) Must be able to qualify for UBH drivers’ insurance coverage
