ICMS Case Manager
Program Type: Intensive Case Management Services (ICMS) — Housing & Supportive Services
Schedule: Monday – Friday | Full-Time (40 hours/week)
We are seeking a motivated and compassionate ICMS Case Manager to join The Good Seed team in Baldwin Hills, CA. This position provides intensive, client-centered case management services to individuals and families experiencing homelessness and housing instability, with a focus on achieving long-term housing stability and self-sufficiency.
The ICMS Case Manager serves as the primary point of contact for participants, providing ongoing outreach, housing navigation, and coordinated support services. This role is grounded in a trauma-informed, strengths-based, and harm-reduction approach to care.
Conduct initial and quarterly psychosocial assessments
Develop and implement individualized service plans in collaboration with participants
Maintain a caseload of approximately 15–20 clients
Support housing stability planning and long-term housing retention
Provide outreach, housing navigation, and housing location services
Conduct in-person visits in community, field, and home settings
Connect participants to healthcare, mental health, and substance use services
Assist with employment, education, and benefits enrollment (e.g., Medi-Cal, GR, SSI)
Accompany participants to medical and social service appointments as needed
Support treatment adherence and care coordination across systems
Utilize harm-reduction, client-centered, and strengths-based approaches
Maintain accurate documentation and case records
Collaborate with multidisciplinary service providers and community partners
Perform other duties as assigned
Required:
Strong commitment to serving individuals experiencing homelessness and housing instability
Knowledge of poverty, homelessness, and related social determinants of health
Understanding of behavioral health, substance use, and disability-related needs
Ability to manage a flexible schedule, including evenings and weekends as needed
Valid California Driver’s License, reliable transportation, and auto insurance
Ability to work independently and in a team-based environment
Strong communication, organizational, and documentation skills
Preferred:
Experience in housing navigation, case management, or social services
Familiarity with Los Angeles County housing and supportive service systems
Experience working with individuals with mental health or substance use challenges
Knowledge of harm-reduction and trauma-informed care models
This position operates in community-based and field settings throughout Baldwin Hills and surrounding areas in Los Angeles, CA. The role requires frequent travel, in-person client engagement, and home visits once participants are housed.
Candidates must be comfortable working in dynamic environments and managing emotionally complex situations while maintaining professionalism, empathy, and a client-centered approach.
