Elementary Education Family Case Manager

Terms of Employment:  12 MONTHS
Exemption Status: NON-EXEMPT
SALARY SCALE: UA G08-09

EDUCATION AND WORK EXPERIENCE
•    Associate’s degree, or combination of education and experience in the field from an accredited college or university, preferred.
•    Bi-lingual preferred.

 

QUALIFICATIONS
•    Ability to work outside regular school hours as needed.
•    Ability to work professionally and discretely with extremely confidential information.
•    Excellent analytical, problem solving and troubleshooting abilities.
•    Proficient in Microsoft Office Suite.
•    Self-motivated with the ability to work both independently and in a team environment.
•    Excellent customer service, communication (both written and verbal), and cross group collaboration skills required.

 

ESSENTIAL JOB FUNCTIONS
•    Coordinates with Judy Center Instructional Specialist to plan and implement programming for children and families within Judy Center catchment areas.
•    Facilitates successful entry students into CCPS by assisting families with the prekindergarten application and kindergarten registration process at Judy Center schools. Will also facilitate introducing EL students and parents to the school setting and culture.
•    Serves as coordinator for families to (1) help their children improve their academic achievement and (2) become active participants in the education of their children through Judy Center programs, school meetings, and workshops with parents.
•    Facilitates the collaboration of community resources and promotes family and individual connections for families within Charles County community specifically the Judy Center catchment areas.
•    Research international family needs, develops, and assists during parent workshops/meetings which may be in collaboration with school staff, outreach staff, and/or representatives from community agencies.
•    Collaborates with other Judy Center schools.
•    Facilitates Judy Center programs, events, early childhood conferences/meetings. 
•    Explains and clarifies school policies and procedures such as attendance, student/parent handbook, and other information as it pertains to Judy Center early childhood programs.
•    Facilitates long-term collaboration between the school system and/or community agencies (e.g., health department, library, social services, etc.) the needs and concerns.
•    Participates in appropriate school meetings, attends in-service training, conferences, and workshops.
•    Participate in IEP, SST, and parent/teacher conferences as necessary.
•    Conduct home visits.
•    Actively conduct outreach in order to identify children 0-4 within Judy Center catchment before they enter school
•    Collaborate with teachers to select and obtain referrals and consents for children for Judy Center tutoring, behavior services and summer programs.
•    Attend health/dental appointments with families and communicate with families to ensure health services are complete.

 

ENVIRONMENTAL FACTORS & PHYSICAL REQUIREMENTS
The environmental factors and/or physical requirements of this position include the following: While performing the duties of this job, the employee is required to have ordinary ambulatory skills sufficient to visit other locations; and the ability to stand, walk, stoop, kneel, crouch, and manipulate (lift, carry, move) light to medium weights of 10-50 pounds. Requires good hand-eye coordination, arm, hand and finger dexterity, including ability to grasp, and visual acuity to use a keyboard. The employee frequently is required to sit, reach with hands and arms, talk and hear.
