Qualifications:

• Valid NY State RN license with a BSN degree
• Proven experience in Intake or CHHA roles
• Strong knowledge of the Home Care Homebase (HCHB) system
• Familiarity with Allscripts and Careport platforms
• Excellent communication and organizational skills
• Compassionate approach to patient care and coordination
