What does a Family Tree Private Lead Community Care Provider do?

  • Provides personal assistance and health-related tasks per the Individualized Service Plan.
  • Maintains a clean, safe, and comfortable environment.
  • Practices infection control in accordance with agency policy.
  • Respects client privacy, dignity, and property.
  • Observes and reports changes in client condition to the office.
  • Schedules caregivers and ensures full shift coverage; monitors schedules daily and address open shifts.
  • Adds requested shifts and communicates changes to office and client/POA; provides updates to clients/POAs regarding scheduling changes.
  • Verifies completion of required daily tasks in WellSky and report gaps;
  • Ensures that care tasks are documented accurately and timely in WellSky
  • Trains and coaches’ new caregivers while escalating performance concerns to Market Director.
  • Serves as backup caregiver as needed to ensure continuity of care.
  • Meets with new clients to assess needs and establish schedules.
  • Conducts post-hospital visits to resume or adjust services.
  • Educates clients and families on available services.
  • Participates in the community’s scheduled activities to promote services.
  • Demonstrates effective time management and teamwork.
  • Attend training courses required to maintain competency and compliance with policies and local/state/federal regulations.
  • Supports agency operations and upholds policies and standards.
