A healthcare organization is seeking compassionate and dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide one-on-one care in a home-based setting. Provide direct nursing care within the scope of licensure and professional standards
Monitor patient conditions and communicate changes as needed
Maintain accurate and timely documentation
Educate patients and caregivers regarding care needs and treatment plans
Collaborate with healthcare team members to support quality patient outcomes
Follow established policies, procedures, and regulatory guidelines
Participate in ongoing training and professional development activities
Active RN or LPN license required
Current CPR certification required
Previous nursing experience preferred
