Responsibilities • Provides direct patient care under the supervision of a licensed provider or RN in accordance with nursing standards and the scope of practice defined by the State Board of Nursing. • Obtains and documents patient vital signs, height, weight, and medical history accurately in the electronic health record (EHR). • Administers medications, injections, and immunizations as ordered, following safety protocols and documentation procedures. • Performs basic wound care, specimen collection, and point-of-care testing (POCT) such as glucose, urinalysis, and pregnancy tests. • Assists with diagnostic procedures and examinations, preparing and positioning patients as necessary. • Supports providers during procedures by preparing instruments, supplies, and maintaining sterile techniques. • Monitors patients for changes in condition and reports significant findings promptly to the RN or provider. • Reinforces patient education on medications, treatments, and lifestyle instructions as directed by providers or RNs. • Supports clinic flow by preparing exam rooms, managing room turnover, and restocking supplies. • Schedules patient appointments, follow-up visits, and diagnostic tests as needed. • Communicates effectively with patients, families, and interdisciplinary team members to coordinate care and ensure seamless patient care. • Participates in patient outreach activities, including follow-up calls, medication refills, and preventive health reminders. • Maintains accurate and timely documentation in the EHR for all care provided. • Ensures that infection prevention standards, safety procedures, and equipment maintenance are consistently followed. • Adheres to HIPAA regulations and organizational policies related to patient privacy and information security. • Maintains knowledge of and compliance with OSHA, Joint Commission, and state/federal healthcare regulations. • Demonstrates professionalism, accountability, and commitment to organizational values in all interactions. • Participates in required training, continuing education, and competency assessments to maintain licensure and clinical proficiency. • Supports quality improvement and patient safety initiatives within the department. • Demonstrate excellent guest service to internal team members and patients. • Performs other related duties as assigned. Qualifications • High school diploma or equivalent. • Graduate of an accredited Licensed Practical Nurse (LPN) program required. • 1 + year of clinical nursing experience preferred; new graduates considered with appropriate orientation. • Experience in ambulatory care, physician office, or acute care setting preferred. • Familiarity with EHR systems (e.g., Epic, Cerner, or similar) preferred. • Strong understanding of nursing principles, clinical procedures, and infection control practices. • Ability to accurately assess, monitor, and document patient condition and care activities. • Excellent communication and interpersonal skills with patients, families, and team members. • Ability to adapt to different clinical settings and patient populations. • Strong organizational and time management skills; able to manage competing priorities. • Commitment to compassionate, high-quality, patient-centered care. • Competent in safe medication administration and clinical documentation. • Demonstrates compassion, empathy, and respect in all interactions with patients. • Applies sound clinical judgment and follows best practices for nursing interventions. • Adheres to established protocols and regulatory requirements to ensure a safe environment. • Builds positive working relationships and communicates effectively with interdisciplinary teams. • Responds to changing patient needs, priorities, and clinical environments with flexibility. • Takes ownership of actions, documentation, and professional development. • Maintains clear, timely, and professional communication with staff, patients, and providers. • May require evening, weekend, or holiday shifts depending on departmental needs. • Ability to work independently in a fast-paced, cross-functional environment.
