Licensed Practical Nurse (LPN) - Care Services Coordinator

Fresh Perspective Home Care is seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team in a hybrid role supporting both field and administrative operations. 

This position  works directly with nursing clients and caregivers by conducting client assessments, developing and updating care plans, providing caregiver training, completing performance evaluations, and ensuring exceptional quality of care.

Responsibilities:

• Provide nursing oversight and support for clients receiving skilled care services
• Conduct initial and ongoing client assessments, including required reassessments
• Develop, review, and update individualized care plans
• Provide direct nursing services to clients as needed
• Perform caregiver performance evaluations and quality assurance visits
• Train, coach, and support caregivers through hands-on and ongoing education
• Review caregiver documentation for accuracy and identify areas of concern
• Monitor client satisfaction through check-in calls, service reviews, and surveys
• Address client and caregiver concerns promptly and professionally
• Maintain accurate documentation and ensure effective communication between clients, caregivers, and office staff
• Collaborate with the Director of Nursing and interdisciplinary team to ensure quality care and compliance with agency standards

• Active LPN license in good standing
• Home Care, care coordination, or case management experience preferred
• Strong communication, organizational, and problem-solving skills
• Valid driver’s license, car insurance, and reliable transportation
• Ability to lift, transfer, and assist up to 40 lbs
• Ability to uphold client confidentiality
• Ability to manage multiple priorities in a fast-paced environment
