We are currently seeking a PRN Licensed Practical Nurse (LPN) to join our hospice team. This role offers the opportunity to build meaningful relationships while delivering hands-on care, clinical management, and education to patients and families during an important chapter of life.

Key Responsibilities:

Provide routine nursing visits and direct patient care

Establish professional relationships with patients, families, and interdisciplinary care teams

Continually assess and evaluate patients' needs and conditions

Educate patients and caregivers about their conditions and care plans

Maintain accurate and timely documentation and charting

Requirements:

Valid Ohio LPN license

Minimum 2 years of nursing experience

Strong clinical assessment and communication skills

Compassionate, patient, and flexible approach to care

Ability to lift/transfer patients as needed

Proficient with computer documentation

Valid driver’s license and one-year clean driving record

Weekend availability is a plus
