Licensed Practical Nurse (LPN/LVN) Acute Rehab

 

Job Type: Full-Time | Nights

 

Schedule: 3x12, Nights (6p-6a), Rotating Weekends

How you’ll contribute 

Licensed Practical Nurse (LPN) who excels in this role provides direct patient care through assessments, nursing interventions, documentation, and interdisciplinary coordination, ensuring the highest standards of safety and quality as well as:

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.

What we’re looking for 

Applicants should hold a current state LPN/LVN license and a graduate of an accredited LPN/LVN program. Additional requirements include:

  • CPR/BLS certification required

  • IV Certification highly preferred or must be obtained within 6 months. 

  • Minimum six months of Medical/Surgical experience in an acute care setting preferred.
