Job Description - Licensed Practical Nurse, Team Leader, Skilled


Full-Time position available with a complete benefits package (Including Medical, Dental, Vision, Paid Time Off, 403B with Match, and more). We are seeking an LPN Team Leader for Skilled Care to join our team.

The LPN/RN team leader for the skilled household provides nursing care to residents in our skilled unit under the physician's orders for short-term rehabilitation patients. 

  • Plan, provide and document advanced professional nursing care in resident care areas to include collecting and analyzing resident data, developing and implementing nursing care plans, responding to emergency and specialized resident care situations, evaluating nursing care practices and communicating to appropriate personnel relevant facts about residents, nursing services rendered and resident responses
  • Assist staff personnel in developing professional skills and in selecting priorities for the delivery of nursing care; monitor progress of staff and assist with reorganization of units as required.
  • Administer medication and treatments such as IV's, NG Tubes, obtains blood samples, and maintain accurate resident charts. 
  • Coordinate activities of nursing staff with other personnel and services engaged in caring for residents; communicate with physicians and advise other services of conditions or circumstances affecting resident care.
  • Supervise a variety of personnel actions to include, but not limited to, consultation with shift supervisor for merit recommendations, promotions, transfers, vacation schedules, sick leaves and dismissals for employees of unit.
  • Plan and provide nursing care under the direction of the Team Leader and in accordance with physician’s orders and established nursing care policies, procedures, and standards.
  • Evaluate nursing care with emphasis on medication administration, prescribed treatments and patient responses; consult with supervisory nurse in the establishment of short-term and long-range goals for patients.

Requirements

  • A valid, unencumbered Virginia LPN license.
  • Two to three years of experience working in skilled or long-term care preferred. 
  • Two to three years of supervisory experience preferred, but not required.
