Home Health Licensed Vocational Nurse (Part-Time, per Diem)

Mare Island Home Health Inc. currently provides services in the following counties:

• Solano County


• Napa County


• Yolo County


• Contra Costa County

Initial Requirements:

• Current license to practice in CA


• Current CPR certification


• Current CA driver's license plus proof of car insurance.


• Minimum one year RN clinical experience (two+ years preferred)


• Computer/Internet experience (online charting to be used)


• Bi/Multi-lingual skills a plus (e.g. Spanish, Russian, Tagalog, etc.)

Preferred Experience:

• LVN in an acute care setting: 1 year preferred

Responsibilities

• Provide high-quality patient care in a home health setting, utilizing core nursing skills such as catheterization, IV insertion, and patient assessment.


• Utilize premium skills like critical care experience and medical office knowledge to deliver specialized care to patients.


• Demonstrate proficiency in relevant skills such as health coaching and specimen collection to enhance patient outcomes.


• Collaborate with interdisciplinary teams to ensure comprehensive and individualized care plans for patients.


• Utilize EMR and EHR systems effectively to document patient information and track progress accurately.
