Full-Time Licensed Vocational Nurse (LVN) – Day Shift

We are seeking a dependable and compassionate Licensed Vocational Nurse (LVN) to join our nursing team at our substance use disorder treatment facility in San Diego, California.

This is a full-time day-shift position, and candidates must be available to work weekends as part of their regular schedule.

The LVN is responsible for supporting the medical and nursing needs of clients receiving detoxification and residential treatment. 

This position requires strong clinical judgment, accurate documentation, medication management, effective communication, and the ability to work collaboratively with the interdisciplinary treatment team.

Essential Duties and Responsibilities

  • Complete all nursing duties and documentation in accordance with facility policies and procedures.
  • Complete and document client rounds as required.
  • Conduct medication and controlled-substance counts according to facility procedures.
  • Review the shift log at the beginning of each shift and complete required shift documentation and assigned tasks before the end of the shift.
  • Complete nursing admission procedures for new clients, including UA testing, vital signs, medication reconciliation, and comprehensive nursing assessments.
  • Complete accurate and timely nursing progress notes and other required documentation in the EMR/EHR.
  • Observe and document medication administration in accordance with facility policies and provider orders.
  • Obtain and document blood pressure, temperature, pulse, respirations, oxygen saturation, and other required vital signs.
  • Complete daily nursing assessments for clients receiving detoxification services.
  • Complete CIWA-Ar, COWS, and polysubstance withdrawal assessments as ordered and clinically indicated.
  • Monitor clients for changes in withdrawal symptoms, medical status, mental status, and overall condition.
  • Communicate changes in client condition and detoxification status to physicians and other medical providers.
  • Provide medication education and reinforce medication compliance.
  • Coordinate medication refills and pharmacy needs as appropriate.
  • Provide crisis intervention and promptly escalate medical or behavioral concerns.
  • Assist, educate, and supervise Behavioral Health Technicians regarding delegated or appropriate medical-related responsibilities within their role.
  • Collaborate with medical providers, nursing leadership, clinical staff, operations, and other members of the interdisciplinary treatment team.
  • Assist with medical and nursing needs of clients receiving residential services.
  • Maintain professional, timely communication with nursing leadership and the treatment team.

Minimum Qualifications

  • Graduate of an accredited school of nursing.
  • Current, active, and unencumbered California LVN license.
  • Ability to work a full-time day-shift schedule, including weekends.
  • Current First Aid and CPR certification.
  • Knowledge of nursing practices, medication administration, documentation standards, and applicable healthcare regulations.
  • Ability to recognize and communicate changes in a client's medical or behavioral condition.
  • Ability to prioritize responsibilities and work effectively with minimal supervision.
  • Strong written and verbal communication skills.
  • Computer proficiency and experience using EMR/EHR systems.
  • Ability to work independently while functioning effectively as part of an interdisciplinary team.
  • Strong attention to detail and commitment to timely, accurate clinical documentation.

Preferred Qualifications

  • Previous experience in detoxification, substance use disorder treatment, behavioral health, or mental health.
  • Knowledge of addiction, withdrawal management, and evidence-based substance use disorder treatment.
  • Experience completing CIWA-Ar and COWS assessments.
  • Experience working in residential behavioral health or addiction treatment settings.
