Licensed Vocational Nurse (LVN) Home Health

Job Summary
We are seeking a dedicated Licensed Vocational Nurse (LVN) to join our Home Health team. In this role, you will provide high‑quality direct patient care under the supervision of a Registered Nurse (RN). You will follow individualized care plans, administer treatments, document clinical information, and collaborate with the interdisciplinary care team to support positive patient outcomes.

Key Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act.

• Carry out the plan of care developed by the Registered Nurse.

• Complete accurate, timely, and compliant clinical documentation.

• Perform patient observations and collect assessment data to report to the RN or physician.

• Communicate the need for additional home health services (HHA, OT, PT, MSW, ST, Dietician) to the clinical team.

• Utilize medical equipment and supplies safely, efficiently, and according to agency guidelines.

• Participate in ongoing professional development and skills training.

Qualifications

• Current LVN license in California.

• Minimum of 1 years nursing experience preferred (home health or medical/surgical experience strongly valued).

• Knowledge of professional nursing standards and clinical best practices.

• Strong verbal, written, and organizational communication skills.

• Reliable transportation for home visits; valid driver’s license and current auto insurance required.

• Current CPR certification.

• Ability to lift, position, and transfer patients; requires frequent standing, walking, bending, and reaching.
