Position Summary
This full-time, standard hours position of Specialist reports to the Director of Family Support Services. This position is responsible for all case management related to the individuals served by the LifeSharing through Family Living program and for ensuring that the families contracted to provide LifeSharing meet all standards as established by the Pennsylvania State Office of Developmental Programs and the contracting agency, as well as the policies and procedures of ProBerco. The Specialist serves as a best practice resource to the families providing LifeSharing and the individuals served by the program. The Specialist assesses and approves families to be LifeSharing providers and matches individuals with families. The Specialist assesses the placements on an ongoing formal and informal basis.
Minimum Qualifications
1. Bachelor's Degree in Special Education, Education, Psychology, or related field; or high school diploma and extensive experience working with persons with physical and intellectual disabilities.
2. Ability to interact with and relate to persons with physical and intellectual disabilities.
3. Demonstrated leadership abilities.
4. Experience with designing and implementing outcomes.
5. Experience with skill assessment instruments.
6. Excellent verbal and written communication skills.
7. Strong organizational ability and the ability to work independently.
8. Ability to represent ProBerco in a professional manner in all settings.
9. Possess a valid PA driver’s license.
Duties and Responsiblities
1. Screen and approve prospective providers.
2. Assist the Director in evaluating and approving prospective participants in the
LifeSharing through Family Living program.
3. Act as a liaison with LifeSharing providers and the contracting agency and
Prospectus Berco and other contracted service providers.
4. Assist with the development, implementation, and monitoring of programs
established for individuals in LifeSharing.
5. Ensure regulatory compliance.
6. Assist with medical appointments and transportation as required.
7. Develop and monitor Individualized Service Plans and outcomes.
8. Conduct monitoring visits to provider homes.
9. Arrange pre-placement visits and team meetings.
10. Provide training to provider families as required by regulation and Prospectus
Berco policy.
11. Coordinate respite care as requested by provider families.
12. Facilitate team meetings as required.
13. Attend meetings and trainings.
14. Advocate in accordance with the mission of ProBerco for individuals
receiving services.
15. Meet with individuals receiving services through LifeSharing to assess placement
success and needs.
16. Prepare required reports in requested format.
17. Regular attendance at work.
18. Other duties as assigned.

Physical Requirements and Environment
• The employee is regularly required to stand, walk, and use hands to finger, handle,
or feel; reach with hands and arms; stoop, kneel, crouch; and talk and hear.
• The employee is frequently required to sit.
• The employee is regularly required to climb or balance and taste and smell.
• The employee may be required to occasionally lift and/or move more than 100 lbs.
• Specific vision abilities required by this job include close vision, distance vision,
peripheral vision, depth perception, and the ability to adjust focus.
• While performing the duties of this job the employee is occasionally exposed to wet
and/or humid conditions and other outside weather conditions.
• The noise level in the work environment is usually moderate.
• The responsibilities of this position will be performed in a variety of settings and
under a variety of environmental conditions.
• Most work will be performed in an office of conference room setting. Some work will
be performed off-site in homes of providers, office buildings, or restaurants.
• A small amount of work may be conducted outside.
• Safety precautions are observed in order to reduce incidents of risk factors for
typical office environment which are adequately lighted, heated and ventilated.
Example: avoiding trips and falls, back care, and observing fire regulations.
• Ability to move in and around personal work space and to and from other areas of
the office or building to deliver/retrieve material, attend meetings, or other activities
outside of personal work space with reasonable accommodations if necessary to
perform essential job functions successfully.
