LPN

As an LPN at Acclaim Home Health you will provide high-quality, compassionate care to patients in their homes. Your role focuses on performing nursing assessments, administering medications, monitoring patient progress, and educating patients and their families to support health and recovery in a comfortable, home-based environment.

Responsibilities

• Provide skilled nursing care in accordance with the patient's plan of care.


• Perform health assessments and monitor patient conditions.


• Administer medications and treatments as ordered by physicians.


• Monitor and document vital signs, symptoms, and patient progress.


• Educate patients and family members regarding disease management and care plans.


• Communicate changes in patient condition to the supervising Registered Nurse and physician.


• Maintain accurate and timely clinical documentation.


• Ensure compliance with all state, federal, and agency regulations.


• Participate in interdisciplinary care planning and coordination.

Requirements

• Current Licensed Practical Nurse (LPN) license in the state of Massachusetts.


• Experience in home health nursing or a related clinical setting preferred.


• Strong communication and interpersonal skills.


• Ability to work independently and manage time effectively during home visits.


• Reliable transportation to travel to patient homes
