LPN for Home Care

University Home Care Inc is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality home care services to clients in the Southeast MI Region. Must be willing to travel.  As an LPN for Home Care, you will play a critical role in supporting patients' health and well-being by delivering personalized nursing care in the comfort of their homes.

Responsibilities

• Provide direct nursing care to clients in their homes according to care plans and physician orders.


• Monitor and record patients’ health status and report changes to healthcare providers.


• Administer medications and treatments as prescribed.


• Assist with activities of daily living and educate patients and their families on health management.


• Collaborate with interdisciplinary teams to ensure comprehensive patient care.


• Maintain accurate and timely documentation of care provided.

Requirements

• Valid and current LPN license in the state of Michigan.


• Previous experience in home care or related clinical setting preferred.


• Strong communication and interpersonal skills.


• Ability to work independently and manage time effectively.


• Reliable transportation to travel to patient homes in the Southeast MI Region.
