LPN On Call

Description:
Hospice is a 24/7 service, and so are we. We know our patients don’t stop needing care when the workday ends, which is why we’re looking for an on-call LPN who can step in on the weekends when patients and families need an extra hand. Working alongside our RN Case Managers and on-call team, you’ll provide compassionate, skilled hospice care during those moments when a visit is truly needed. This role is a great fit for an LPN who is confident in their clinical skills, comfortable working independently, and understands that hospice care is about so much more than managing symptoms. It’s about listening, reassuring, educating, and helping patients and families feel supported when they need it most.

Position Requirements: 

• Current LPN license in the State of Georgia


• Current CPR & First Aid Certification


• Reliable and legal transportation to and from patient's place of residence


• Hospice or home care experience is a plus, but kindness and reliability matter the most
