    About the Role


    • Provides coordinated, seamless care for structural heart patients from referral through discharge and follow-up

    • Works collaboratively with cardiologists, cardiovascular surgeons, nurse practitioners, and the structural heart coordinator

    • Supports patient assessment, care planning, and education across the full care continuum

    • Serves as a liaison between patients, families, providers, and referring facilities

    • Promotes continuity of care, compliance, and positive patient outcomes


    How You’ll Drive Our Mission Forward


    • Assist with pre-, peri-, and post-procedure patient care activities

    • Coordinate scheduling, testing, and follow-up appointments to ensure efficient care delivery

    • Act as a primary point of contact for patients, families, and referring providers (including phone triage)

    • Educate patients and families to ensure understanding of treatment plans and available resources

    • Track patients throughout the continuum (pre-admission, inpatient stay, 30-day and 1-year follow-ups)

    • Ensure compliance with Medicare requirements and structural heart registries (e.g., TAVR)

    • Collaborate with multidisciplinary teams to improve care coordination and patient outcomes

    • Support data collection, case reviews, and quality improvement initiatives

    • Build and maintain relationships with referring physicians and external partners


    What Makes You a Great Match


    • Licensed Practical Nurse (LPN) with a current Louisiana license and BLS certification

    • Minimum of 2 years of experience, preferably in a cardiology or clinic setting

    • Strong organizational and multitasking abilities in a fast-paced environment

    • Excellent communication and interpersonal skills with patients, families, and healthcare teams

    • Compassionate, patient-focused approach with emotional intelligence

    • Ability to think critically, solve problems, and make sound decisions

    • Comfortable working independently while collaborating with a multidisciplinary team

    • Knowledge of healthcare regulations, documentation, and patient care standards

    • Skilled in patient education and care coordination

    • Detail-oriented with strong documentation and time management skills
