Position Summary

True Care, a NY LHCSA is seeking an experienced Licensed Practical Nurse (LPN) to supervise clinical review staff and coordinate day-to-day clinical operations for our home care program. Working under the direction of the Director of Patient Services (RN), the LPN Supervisor ensures that clinical records, aide documentation, authorizations, and compliance tasks are complete, accurate, and aligned with New York State Department of Health (NYSDOH) home care regulations, including 10 NYCRR Part 766. This role is ideal for a detail-oriented LPN with strong home care experience and a talent for organizing teams and workflows.

Key Responsibilities

Clinical Record Review & Documentation Oversight

• Supervise and support a team of clinical reviewers and clinical support staff in the review of patient records, aide task sheets, and visit documentation.


• Review documentation for completeness and consistency with the RN-developed plan of care, physician orders, and agency policy.


• Identify gaps or discrepancies and escalate clinical concerns, changes in patient condition, and plan-of-care issues to the supervising RN.


• Conduct routine chart and documentation audits and track corrective follow-up to completion.

Day-to-Day Clinical Operations

• Manage daily workflow, work queues, and staff assignments to meet productivity and turnaround targets.


• Track due dates for RN supervisory visits, reassessments, plan-of-care updates, and physician order renewals, and alert the RN team in advance.


• Coordinate with intake, scheduling, coordination, and MLTC/managed care plan staff on authorizations, service changes, and hour requests.


• Monitor HHAExchange and EVV visit verification exceptions and ensure timely resolution.


• Resolve patient, caregiver, and staff concerns promptly, escalating clinical matters to the RN.

NYSDOH Regulatory Compliance

• Support compliance with NYSDOH home care requirements under 10 NYCRR Part 766 and related agency policies.


• Monitor aide personnel compliance, including Home Care Worker Registry status, Criminal History Record Check (CHRC) clearance, annual health assessments, required immunizations/testing, competency evaluations, and annual in-service hours.


• Maintain survey-ready records and assist with NYSDOH surveys, managed care plan audits, and internal audits.


• Support the agency's Medicaid compliance program, including OMIG requirements, and participate in QAPI activities, incident reporting, and complaint follow-up.


• Protect patient information in accordance with HIPAA and agency confidentiality policies.

Staff Supervision & Development

• Participate in hiring, onboarding, and training of clinical reviewers and clinical support staff.


• Provide day-to-day direction, coaching, and performance feedback to non-RN staff.


• Assist the RN team with coordinating aide in-service training and competency sessions.

Scope of Practice

In accordance with the New York State Education Law and NYSDOH requirements, this position works under the direction of a Registered Nurse. The LPN Supervisor does not perform independent patient assessments, UAS-NY assessments, development of the plan of care, clinical supervision of home health aides, or medical necessity determinations; these remain the responsibility of RN staff.

Qualifications

Required

• Current, unrestricted Licensed Practical Nurse (LPN) license and registration in New York State.


• Minimum 3 years of LPN experience, including at least 2 years in home care (LHCSA, CHHA, or MLTC).


• Minimum 1 year of lead, coordinator, or supervisory experience.


• Working knowledge of NYSDOH home care regulations, aide compliance requirements, and Medicaid/MLTC authorization processes.


• Proficiency with HHAExchange (or similar home care platform), EVV, and Microsoft Office.


• Strong organizational, documentation, and communication skills.

Preferred

• Experience supporting NYSDOH surveys or managed care plan audits.


• Familiarity with the Home Care Worker Registry, CHRC, and aide training program requirements.


• Experience with quality improvement (QAPI) or chart audit programs.


• Bilingual: English/Spanish or other languages common in the NY area.
