Job Description - LTC Community Health Worker

Here’s What You’ll Be Doing:



  • Builds positive, respectful relationships with residents while supporting a safe, structured, and person-centered living environment.

  • Assists residents with activities of daily living (ADLs), including personal care, dressing, grooming, and mobility.

  • Observes resident wellness and communicates changes or concerns to appropriate team members.

  • Collaborates with the House Manager, Lead CHW/Activities Coordinator, and Community Health staff, including RN case managers, to support coordinated care.

  • Supports medication routines in accordance with program guidelines.

  • Supports daily wellness and engagement through recreation, activities, and individualized interactions.

  • Supports development of independent living skills through household routines, including cooking and light housekeeping.

  • Assists with community outings, appointments, and activities that promote inclusion and participation.

  • Documents services provided, resident participation, and observations in the electronic health record (EHR) in accordance with program requirements.


What you’ll need to be successful:



  • High School Diploma or GED required.

  • Nursing Assistant (CNA) experience preferred.

  • Ability to provide direct resident support, including activities of daily living (ADLs).

  • Experience in in-home care, residential care, or similar setting preferred.

  • Experience supporting or leading activities for individuals with varying support needs preferred.

  • Medication administration experience preferred; ability to complete competency evaluation prior to independent medication passing required.

  • Must be 18 years or older and have a valid driver’s license and meet eligibility requirements for Tribal vehicle use.

  • Must successfully pass all applicable background checks and drug screens.

  • (Health Only) FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
