Job Description - LTC Lead Community Worker & Engagement Coordinator
Here’s What You’ll Be Doing:
Builds positive, respectful relationships with residents while supporting a safe, structured, and person-centered living environment.
Assists the Residential Manager with coordination of daily services, resident routines, and activity programming.
Serves as a resource to team members for activity and engagement strategies, implementation, and consistency across shifts.
Develops, plans, and leads structured daily activities, including individual, group, and culturally relevant programming.
Helps to develop and coordinate weekly meal planning, grocery shopping, and meal preparation activities in collaboration with the LTC staff and Community Health Dietician, to support resident participation, life skills development, and healthy routines.
Supports daily wellness and resident engagement through recreation, structured activities, and individualized interactions.
Assists residents with activities of daily living (ADLs), including personal care, dressing, grooming, and mobility as needed.
Supports development of independent living skills through daily routines, including cooking and light housekeeping.
Observes resident wellness and communicates changes or concerns to appropriate team members.
Documents services provided, resident participation, and observations in the electronic health record (EHR) in accordance with program requirements.
What you’ll need to be successful:
Associate’s degree in human services, healthcare, or related required; Bachelor’s degree preferred.
Experience developing, coordinating, and/or implementing culturally relevant programming that reflects Forest County Potawatomi Community values and traditions preferred.
Nursing Assistant (CNA) experience preferred.
Experience in in-home care, residential care, or similar setting preferred.
Experience supporting or leading activities for individuals with varying support needs preferred.
Medication administration experience preferred; ability to complete competency evaluation prior to independent medication passing required.
Must be 21 years or older and have a valid driver’s license and meet eligibility requirements for Tribal vehicle use.
Must successfully pass all applicable background checks and drug screens.
(Health Only) FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
