Position Details

• Schedule: Full-Time
• Annual Salary: $114,400
• Location: Medical Hill Healthcare Center

Key Responsibilities

• Coordinate and oversee resident care plans from admission through discharge.
• Collaborate with physicians, nursing staff, therapists, residents, and families to ensure continuity of care.
• Monitor clinical progress and identify opportunities to improve patient outcomes.
• Facilitate care conferences and interdisciplinary team meetings.
• Assist with discharge planning and transitions of care.
• Ensure compliance with federal, state, and facility regulations.
• Maintain accurate clinical documentation and case management records.
• Advocate for residents while promoting quality care and customer satisfaction.

Qualifications

• Current California LVN license in good standing.
• Case management, skilled nursing, or long-term care experience preferred.
• Strong assessment, communication, and organizational skills.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Knowledge of Medicare, Medi-Cal, managed care, and discharge planning processes is a plus.
