Lvn/Lpt

Under the supervision of the MHSA-FSP Assistant Program Director, the LPT/LVN provides comprehensive medication services to members in the MHSA-FSP program. The LPT/LVN work with and take direction from the psychiatrist(s) in providing medication services to clients. Adheres to all required documentation requirements.

Position Competencies:

• Ability to relate to individuals who have severe impairments in their ability to function in the community.


• Ability to respond quickly, effectively and with equanimity to member crises and life situations.


• Knowledge of Los Angeles County Department of Mental Health’s Rehab Option model of service provision and documentation requirements.


• Basic computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and negotiate through various screens or windows to complete daily chart documentation.


• Ability to work as a team member and to work with other agencies, community resources and member’s families.


• Knowledge of psychotropic medications and their administration, including side effects.

Position Requirements:

• Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.


• Current, valid California Psychiatric Technician License (LPT) or Licensed Vocational Nurse License (LVN) is required.


• An Associate's Degree in mental health or related field in addition to LPT / LVN License is preferred.


• Must have at least two years of experience as a LPT/LVN working with seriously mentally ill population. Experience working with homeless mentally ill and/or dually diagnosed persons is preferred.


• Must have valid California Driver's license and the availability of a car with adequate insurance.

Primary Duties:

• Work as a team with member’s Personal Services Coordinator, psychiatrist and other interagency staff to provide consistent services.


• Enroll new members by locating, working with other service providers, assessing, and introducing the program. Ensure that Consent for services and Transfer of Coordinator forms are completed and submitted in a timely manner.


• Counsels member=s individually and in groups around their medications, side effects and compliance issues. Assist members in developing independent medication management skills.


• Conducts medication group as scheduled weekly.


• Maintain weekly contact with all assigned members. This can be a phone contact if a face-to-face is not possible or indicated. Maintain contact with family members and significant other support persons whenever appropriate.


• Provide mental health rehabilitation services, such as counseling, money management, medication compliance, social skills training, and other independent living skills as needed by assigned members.


• Provide intensive crisis intervention services twenty-four hours, 7 days a week to members at risk of hospitalization and /or incarceration in an effort to avert hospitalization and /or incarceration.


• Involve members in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible.


• Ensure that members are involved in establishing goals in the areas of medication compliance and medical care.


• Maintain a UOS log on a daily basis, documenting all services to be billed and attaching the progress notes for these services. Service codes and time billed must be consistent on the UOS log and progress note. Documentation is to be completed and submitted in a timely manner.


• Responsible for administering I.M. medications as ordered by psychiatrist...


• Administer oral medications as ordered by psychiatrist. Make recommendations to psychiatrist regarding medication changes based on observation of member=s behavior and symptomology.


• Document all medications administered in compliance with agency policy. Dispose of all sharps in compliance with agency policy.


• Enter all medication administered on the appropriate medication log.


• Develop annual and six month service plan aimed toward recovery based on members established Coordination cycle dates.


• Bill at least six hours of services daily.


• Prepare for weekly clinical staff meetings by having current information on the status of all assigned members.


• On an ongoing basis, complete all Rehab Option, Outcomes study and other paperwork within prescribed time frames.
