Marketing - Director of Care Transition

Oxford Health and Rehab is searching for an energetic healthcare marketing professional to serve as Director of Care Transition.  This person must have experience in healthcare marketing, care transition, and working with the elderly population.  The primary responsibilities include promoting our brand and maintaining good public relations for the facility.  The successful candidate will be organized and have excellent logistics and communication skills in order to establish and maintain good relationships with transferring facilities.  This position reviews all referrals and physically assesses patients to determine appropriateness for admission and coordinates the admissions process with all team members.  

Must have a degree in Marketing, Communications or related field OR at least 2 years experience in the healthcare industry working in Marketing or Admissions. Nursing background preferred.
