MDS Nurse

MDS Nurse - Hannah Duston Healthcare Center
Full Time, 40 hours/week 
Base Pay: $33 - $47 (Depending on RN/LPN)


Essential Functions:
- Ensures accurate and timely completion of all resident assessment instruments including MDS sheets, facesheets, trigger sheets, RAP summaries and care plans
- Update staff on new and/or revised laws and interpretations of laws governing resident assessment
- Upon a resident’s admission, assist in initiating the assessment process
- The MDS Nurse will:
o Complete nursing sections of MDS and/or 90 day (quarterly) MDS for all Medicare/managed care patients
o During the comprehensive assessment following admission, circle/highlight appropriate triggers and pull corresponding RAP’s for completion
o Complete nursing RAP’s as needed
o Assist with other MDS’s as needed when Medicare/Managed Care census is low
- Sign the back of each nursing trigger sheet once each trigger has been appropriately addressed, a statement made on each regarding to location of information and date has been addressed
- Assumes the same responsibilities per MDS assessment schedule, but at least monthly
- Tracks consistency between documentation

Prerequisites:
- Registered Nurse or Licensed Practical Nurse by the Commonwealth of Massachusetts
- Two (2) years previous experience in long term care
- Knowledge of DPH, OBRA, JCAHO, Medicare regulations and MDS process
- Experience working within interdisciplinary team approach to care
- Computer skill required
- Knowledge of growth and development across the life span required
