Job Description - Med Tech


JOB TITLE: MED TECH / PERSONAL CARE GIVER OR CNA (DIRECT CARE STAFF)

REPORTS TO: Wellness Manager/ Administrator

FLSA STATUS: Non- Exempt

ESSENTIAL JOB FUNCTIONS:

The Med Tech/PC-CNA will work under the direction of the Administrator and Wellness Manager to ensure that quality service is provided to all residents while functioning within the guidelines of regulation and policy.

  • Assist in maintaining resident care (ADL’s, supervision, safety, documentation, activity, escorting)
  • Complete medication management and administration per Title 24 regulations and company policy. 
  • Communicate & report regularly to the supervisor and administrator & provide status reports for resident care.
  • Understand and follow each shift’s specific assignments, in addition to the regular routine.

HANDLING CRISIS: CALL 911 for any life-threatening emergency.

  • Med Tech /PC on duty is in charge to address all emergency situations, call 911 for any emergency, life threatening or injury to resident or staff.
  • Conduct complete assessment of the situation and make suitable judgement call which may change based upon the nature of the crisis. 
  • Knowledge of Company’s Emergency and Non-emergency and reporting policy.
  • Provide basic emergency care as determined by the needs of the residents by providing basic first aid.
  • Notify: Immediately contact Administrator or supervisor to notify about the situation. 
  • Crisis Plan understand your responsibilities and respond to crisis. Work with the available team and ask for assistance from all departments. 
  • Tools: Get access to the contact lists, emergency supplies and take actions to provide safe, comfortable environment for residents and staff.

Med tech/PC role in absence of Wellness manager and the Administrator

  • Exercises a high level of independent judgments to determine appropriate response to emergency situations. 
  • Upon any injury or illness, attend and assess the resident or staff, find effective solution take actions per company protocols.
  • Cover the med room until another med tech arrives to take charge, notify supervisor if meals and breaks were missed or interrupted for work related matters.
  • Always execute the safety policy and enforce resident care and supervision.
  • Maintain teamwork, be polite, courteous and use effective positive communication, Avoid any conflicts with coworkers or other department employees. Med techs on shift are not authorized to counsel, discipline any staff.
  • For any occurrence or issue, notify and make report for the Administrator.

Oversee and follow medication services as follows:

  • Maintain a neat and orderly medication room.
  • Maintain current and accurate resident charts.
  • Order medications as needed per facility policy and regulation.
  • Assist with new or refill medication (routine, PRN, narcotics) before it runs out.
  • Set up for medication pass per regulations.
  • Count and maintain narcotics sign in log per regulation
  • Assist residents with self-administration of medications including orals, liquids, patches, inhalers, drops, ointments, treatments. Administration of Injections by CRV staff are prohibited.
  • Sign EMARS (digital medical record) right after administering medication as per the requirements.
  • Check regularly for expired medications.
  • Follow medication destruction regulations.

Assist residents with activities of daily living (ADL) as listed on the residents Service Care Plan.

These activities may include ambulation assistance, lifting, transferring, bathing, dressing, skin care, personal care, mouth/denture care, hair care/shampoo fingernail/food care, shaving, personal Hygiene, dining, incontinence care, making beds and other daily tasks, as needed.

  1. Respond to call lights within 1 minute and provide reason if cannot arrive to resident’s room within given time.
  2. Make resident’s room visits, maintain “hourly check” logs, and initial the log after making rounds.
  3. Monitor skin conditions and changes and report to Supervisor and the Administrator.
  4. Provide incontinent care to the resident, including the proper use of incontinent briefs; reinforcing bowel and bladder training; incontinent discard policy by using designated pails. Assist residents with toileting, including the use of urinals, commodes, and toilet seat extensions. Never throw soiled incontinent in toilets, discard in the designated Incontinent Pails.
  5. Follows two persons assistance for incontinent care if resident is aggressive or not compliant.
  6. Apply principles of basic nutrition to resident care by identifying therapeutic diets; recognizing personal, cultural and religious variations in diet; understanding the dietary problems of aging; provide meals and fluids as Doctors order; prepare and position the resident for meals; assist the resident with eating using effective feeding techniques.
  7. Monitor Dining room census log. Promptly report to the Wellness Dept or Administrator if resident has missed meals, eating less, refusing meals, difficulty swallowing, change in diet etc.
  8. Assure that residents are properly and comfortably clothed. Room temperatures are comfortable.
  9. Make residents beds, prepare their personal inventory, organize their closets, and neatly store personal items in designated place.
  10. Observe and report resident’s physical, mental and emotional condition changes, and report promptly.
  11. Respond effectively to resident behavior. Use effective communication skills with residents who have sensory deficits and or dementia.
  12. To maintain internal communication, utilize two ways radios (walkie –talkies) and follow the protocols of sign- in, Charging units at end of the shift. Do not mute the walkie talkie during your shift (except when taking breaks)

DOCUMENTATION:

  • Utilize the designated forms, templates and logs approved by the company.
  • Read the communication log at beginning of shift read and respond with your follow up notes. 
  • Report change of condition, falls, incidents, and deaths to the med tech on shift & the Administrator 
  • Use Physicians Visit Form to notify the doctor of resident’s change of conditions or decline in health or to document change of medications, refusal of medication etc. Assist visiting nurses, physicians during site visits. 
  • Assist receptionist in scheduling medical appointments and maintain a current medical appointment calendar.
  • Follow up on doctor’s orders and make required changes in medications or dosages, 
  • Follow thoroughly the discharge notes of resident returning from hospital or skilled/rehab
  • Prepare incident reports and submit to the administrator.
  • Document in the Narrative Notes -incidents, falls, hospitalizations, and notes of communication with family.
  • Update Resident Charts, Physicians Report, Face sheets, Emergency files. Maintain and update Resident Info logs like contact, insurance, doctor list, allergy list, diabetic list etc. 
  • Ensure DNR (if applicable) are signed by physicians. DNR stickers are on the residents’ chart and emergency document package. Prepare and maintain all documents in labeled binders, keep blank templates in designated drawer.
  • Document on paper MARS, if the electronic EMARS is not functioning for whatever reason or the internet is down. Maintain manual copy for records. 
  • To maintain accuracy and avoid med error, match the EMARS with Drs order and Bubble package prior to preparing and pouring medications. 
  •  For late documentation on the medical records or the safety check log, always acknowledge this by stating "written in retrospect". Making an advance entry on the Medical Records and or on the Safety Check logs in advance is prohibited. (Advance documenting initials on Medical records (EMARS) before resident was administered the meds or initializing safety check logs without physically checking upon the resident at the scheduled time are considered Falsification of documents and anyone who has been found to have engaged in this activity will be immediately terminated from employment).

GENERAL DUTIES:

  • Orient new residents to the community and follow “New resident Orientation Protocol”.
  • Respond effectively to resident behavior. Use supportive communication in resident interactions including effective communication skills with residents who have sensory deficits or dementia.
  • Support the mental health and social service needs of residents by: observing and reporting mental status and behavior changes; assist and instruct residents to be independent in the activities of daily living.
  • Maintain emergency supplies, Incontinent Supplies (gloves, pads, diapers, wipes, incontinent pail refill bags) linens, blankets, janitorial supplies in the med room and med room storage. 
  • Order and maintain inventory of all supplies, update disaster kit and refill first aid kits as needed
  • Understand instructions to attend to minor room issues like turning on/off air-condition, heater, thermostat, TV/Cable etc. to provide concierge services to resident’s after business hours.

CUSTOMER SERVICE & HOSPITALITY:

  • Greet residents, family members, guests, visitors, tours with a smile and make eye contact & ask how you can help.
  • Follow company’s hospice policy, initiate, and coordinate with Kitchen staff to offer meals/beverages for the guests visiting the actively dying hospice resident.

ER POLICY:

  • Report any emergency, falls, incidents to Med tech on duty and administrator. Call 911 for any life threatening or emergency matters.
  • Provide basic emergency care as determined by the needs of the residents by providing basic First aid. It is not mandated to provide CPR but if employees choose to perform CPR, then must have a current CPR certificate and check the DNR status. Before starting CPR, first ensure the person is unresponsive and once start CPR do not stop till paramedics arrive.

SAFETY & SECURITY POLICY:

  • Leave the Med room Key set at the premises with next Med tech on shift.
  • PM and Night duty are the awake shifts and are responsible to make rounds & lock all first-floor doors by 8:00 pm. Supervise, monitor safety and security of residents, answer resident calls, make rounds every hour, maintain scheduled safety checks, change incontinent, handle incidents and any resident issues.    
  • Have knowledge of transfer devices, provide safe transfers, positioning and turning of residents using effective body mechanics and follow proper lifting techniques per company policy of two persons lift. (Violation of the safety policy of lifting or transferring resident by one person will lead to disciplinary action leading to termination). 
  • Follow daily Hoyer lift safety practice and report to maintenance ASAP if find any malfunction in the Hoyer lift. 
  • Staff must wear back supports when lifting/transferring residents and nonskid shoes during their shifts. 
  • Practice good infection control procedures by using appropriate hand washing techniques. Wear disposable gloves whenever handling resident care and not to use same gloves when handling next resident. Remove and discard gloves before leaving Residents room. 
  • Follow the Waste discard protocols for regulated waste (i.e. containing liquid or semi-liquid blood or other potentially infectious materials; sharp needles) and the conventional waste (i.e. non-fluid blood unsaturated blood-stained Band-Aids, bandages, soiled sanitary napkins, gauze, cotton swabs).
  • Maintain a safe environment by: keeping the community safe, clean, odor free and comfortable; recognizing environmental safety Hazards, fall risk, understand the methods to prevent on-the-job accidents and injuries; using safe practices with O2 is in use.
  • Identifying and follow the community's emergency protocols, disaster plans; pandemic prevention and follow the community's fire procedure, including the operation of fire extinguishers & utilities shut off.                   
  • Report any repairs or safety hazards to the administrator and document it in the maintenance log. 
  • Intervene in situations involving unsafe or inadequate care and report immediately to the Administrator or supervisor.
  • Initiate the Check-out and Check-in process when Resident is leaving and returning to the facility from a short or extended stay.
  • Upon residents return from hospital the med tech on shift need to attend and reassess the resident.
  • Follow directions from discharge notes and communicate with all other departments in regards to resident care needs. 
  • Schedule a follow up with residents’ primary physician within same week of discharge from the hospital.

Employees’ responsibility for Elopement Prevention:

  • Conduct assessments of the residents reported to be confused, maniac, aggressive, paces, seeks exits.
  • Notify the change of condition to the physician, responsible representative, and coordinate a care plan meeting to find out the best solution for residents seeking exit or pacing.
  • The Med tech /PC monitor resident’s attendance during each mealtime using Dining Room Census Log.
  • Ensure all assigned direct care staff are conducting the hourly safety checks.
  • Have knowledge of resident’s care plan and be aware of the residents who are not to leave community without assigned staff or designee. (CRV staff/ private care giver/ responsible person/ third party services).
  • Follow Wander guard policy (if applicable for SCU & selective AL residents who may be flight risk).
  •   Prior to leaving the Secure Care unit, each time the CRV assigned staff must sign the SCU outing log (Neon Green Binder) and Front office outing log and both care giver and receptionist together count the residents. Upon returning Assigned staff needs to follow the same routine and formally handover SCU resident/s to Secure Care staff.
  • Violation of the “Elopement” (if resident is missing from the facility) policy will lead to disciplinary action up to, and including, termination, and further legal action may be taken by the company, as necessary.

Rules & Regulations:

  • Report to work on scheduled time. (Late arrival, early departure, or other unanticipated and unapproved absences from the scheduled hours are disruptive and will lead to disciplinary action up to termination).
  • Follow posted meal and rest break schedules and clock in/ out for shift & meal periods. Notify business office if your meal period or rest break was interrupted, delayed or missed due to work related reasons.
  • Attend all scheduled Care team meetings, and all mandatory in-service meetings and trainings.
  • Follow the “Med Techs Training Manual” and ongoing training & in-service protocols. 
  • Follow applicable laws, rules and regulations relating to Residential Care Facilities for the Elderly ("RCFE"), Including, Title 22, Division 6, pertaining to Abuse of Elders and Dependent Adults, HIPPA/ Confidentiality, privacy law, resident rights, promote dignity.
  • Works as a team player with all coworkers, & management team and extend great service to the Residents.
  • Develop and maintain positive working relationships with physicians, visiting nurses, resident family, private caregivers
  • Complete assigned duties and perform within established procedures and guidelines.
  • Follow standard code of conduct, understand company’s harassment, discrimination and bully policy.
  • Follow ALL company rules, regulations, and policies.

Requirements

MINIMUM QUALIFICATIONS:

  • One years’ experience in Healthcare positions.
  • Possess excellent written and oral communication skills, & the ability to speak and write in English.
  • Knowledge of the requirements for providing care and supervision appropriate to the residents
  • Ability to supervise the resident and medication management within established guidelines & procedures.
  • Enthusiastic, well organized with exceptional listening skills and compassion for the elderly.
  • Demonstrate a professional demeanor, teamwork, multi-tasking, basic computer knowledge and skills.
  • Title 22 required current 16 hours certification and continued 8 hours annual med tech training.
  • Has current Adult First Aid card /CPR upon hire and maintains current status throughout employment.
  • This position requires multiple tasking, hospitality, customer service, flexibility to cover shifts /call offs and cross training/ rotation between the AL and SCU resident care assignments, medication pass and “fill in” roles outside their primary job responsibilities (delegated by the Supervisor or the Dept Lead).

PHYSICAL REQUIREMENTS:

In a typical (8) hour shift, Med tech/PC-CNA will routinely be required to stoop, carry, bend, squat, kneel, crouch, climb, grip, reach, lift, push and pull. Med Tech/PC-CNA will also occasionally be required to lift overhead, waist high and floor level. The maximum lift will be (50) pounds. Company requires always two persons assistance for lifting or transferring resident. Med tech will also be required to use fine motor and manual dexterity skills in using walkie talkies, writing and grasping. MED TECH/PC- CNA may be subject to Potential exposure to infectious waste, disease and conditions etc., including TB, HIV, and Hepatitis B.
