Work Schedule: Monday-Friday 9:00 AM - 5:00 PM
Location: Onsite | Brooklyn, NY 11218
Key Responsibilities:
Collaborate with Clinical and Operational management to lead team of Medical Assistants
Assist with patient and medical assistant team oversight of Chronic Care Management (CCM), Physical and Rehabilitation Medicine (PRM), Behavioral Health Integration (BHI), and other POD programs
Assist with oversight of Medical Assistant documentation, workflow and metrics
Monitor and review patient vital signs, alerts and other concerns
Answers phones, respond to inquiries and triage calls appropriately
Assist in the development and implementation of individualized care plans
Respond to urgent care needs when necessary
Assist with administrative tasks
Screen patients and make referrals accordingly to other clinical lines
Timely completion of clinical requests and follow ups
Documentation audit reviews
Maintain comprehensive medical records of all patient interactions
Perform other related duties as needed
Qualifications:
Fluency in Chinese is required.
High School Diploma or equivalent GED; completion of a Medical Assistant program or equivalent clinical training is strongly preferred.
Excellent communication, interpersonal, organizational and time management skills.
Ability to work independently and collaboratively within a team environment.
Experience utilizing EMR systems and maintaining accurate clinical documentation.
Compassionate, patient- focused, and committed to providing high-quality care.
Knowledge of complying with all state requirements and facility policies and procedures.
Extensive experience working in a clinic, hospital, or multi-specialty medical practice setting is preferred.
Previous leadership, supervisory, or team lead experience in a healthcare environment is strongly preferred.
Demonstrated ability to support clinical operations, coordinate workflows, and assist in guiding team members within a fast-paced medical environment.
