Schedule: Full Time, Monday-Friday 10:00am-6:30pm

Position Summary

The Medical Coordinator supports medical and medication operations in collaboration with the medical provider, nurse, and treatment team. Responsibilities include coordinating primary care appointments, pharmacy coordination and medication pick-ups, monitoring vital signs, documenting medical and medication-related information in QuickMAR and UWITS, supporting communication and care coordination with internal and external providers, transporting clients to appointments as needed, providing updates to the treatment team, and monitoring clients following medication passes and injections.

Essential Duties 

• Establish primary care appointments for newly admitted clients and provide updates on medical needs, appointment follow up’s and progress during treatment team meetings.
• Monitor client medications during medication passes when the nurse is not available.
• Complete client vitals as directed and report on any concerns. Adding clients to the “additional monitoring” portion of the shift notes if needed for medical monitoring. 
• Assist with picking up newly prescribed medications as needed, promptly communicate any discrepancies, concerns, or client-related issues to the medical provider/team.
• Coordinate, track, and maintain follow-up medical appointments on the client appointment calendar with internal and external providers, ensuring clients are informed and prepared. Assist with transportation and share follow up information with the treatment team weekly. 
• As assigned by the nurse, communicate with VOA programs, clinics, hospitals, medical/dental providers, and pharmacies to coordinate medications, records, referrals, and continuity of care.
• Transport clients as directed by the nurse 
• Assist nursing team to maintain proper patient medication storage.
• Maintain facility emergency kits with adequate medical supplies.
• Work with staff manager bi-weekly to dispose of medications left behind or that need to be discarded according to VOA policy.
• Record keeping. Ensure all documentation and paperwork related to client appointments and medications are completed properly and in a timely manner.
• Ensure an incident/critical incident report is completed on specific emergency incidents by the end of shift.
• Attend work as scheduled and arrive at the designated time.  
• Work must be performed on-site due to the need to provide direct client care. 

Secondary Duties 

• Follow all licensing and health department rules and regulations.
• Maintain positive, professional interactions with community resources 
• Attend daily team meetings and weekly treatment teams. Attending all other staff meetings and training courses as assigned.
• Utilize de-escalation skills to manage client crisis by adhering to the De-escalation Training. Utilize supervisory support as needed.
• Perform other functions as necessary or assigned.

Education/Experience/Qualifications

• High school diploma or GED required. 
• Within the first 30 days will need to complete medication monitoring training, QuickMar, and UWITS training.    
• Certified Nursing Assistant (CNA), Medical Assistant (MA), or similar healthcare credential preferred; or a minimum of one to two years of experience in healthcare, behavioral health, residential treatment, a medical office, or care coordination preferred.   
• Experience working with individuals with mental health and/or substance use disorders preferred. 
• Current CPR and First Aid certification required. 
• Demonstrated ability to organize, prioritize, and successfully manage multiple tasks while maintaining a high level of attention to detail. 
• Computer proficiency, including Microsoft Office and the ability to learn and utilize electronic health records and medication management systems. 
• Ability to maintain professional boundaries and confidentiality and comply with applicable regulations, agency policies, and procedures. Ability to interact professionally and effectively with a diverse workforce, clients, healthcare providers, and members of the public. 
• Willingness and ability to accept supervision, direction, feedback, and ongoing training. 
• Must be at least 21 years of age and possess a valid driver's license, maintain a satisfactory driving record, and meet agency insurability requirements. 
• Must be able to successfully complete the Utah Department of Health and Human Services Office of Licensing background screening and pre-employment drug screening requirements.

Physical Demands

• Ability to lift and carry up to 25 pounds. 
• Ability to walk, stand, and move throughout the facility and community for extended periods of time. 
• Ability to walk up and down stairs. 
• Ability to safely assist with transporting and accompanying clients to medical, dental, pharmacy, and other healthcare-related appointments. 
• Ability to travel in agency vehicles and/or utilize public transportation, including walking to and from transit stops and navigating public transportation routes with clients. 
• Ability to enter and exit vehicles and accompany clients to appointments and community locations. 
• Work is performed in a variety of settings, including office, residential, healthcare, and community environments. 
• Ability to use a computer and other electronic systems for extended periods of time to complete documentation, scheduling, care coordination, and other assigned duties.
