Chapa-De Indian Health is hiring its next Medical Director.

Chapa-De is a nonprofit tribal health organization and Federally Qualified Health Center that has served Placer and Nevada counties for nearly 50 years, with health centers in Auburn and Grass Valley and a $58 million third center opening in Rocklin in March 2027. The sitting Medical Director has been promoted into the organization's first Chief Clinical Officer role, and this seat reports to her. It is a succession hire: the provider group, the leadership team, and the program you would lead are already in place and working.

The seat

• Two days a week of patient care (16 to 18 patients a day on 20-minute visits, with a dedicated MA and ambient AI scribing) and two and a half to three days of clinical leadership, on a 36- or 40-hour week
• A small panel, capped around 450 to 480 patients, sized to the two clinical days
• Clinical leadership of about 16 physicians and family nurse practitioners across Auburn, Grass Valley and, from March 2027, Rocklin, with the group growing as the new center opens
• Hiring manager for physicians, and a standing part of every nurse practitioner discovery call and first-round interview
• Member of the organization's leadership team, reporting to the Chief Clinical Officer; the Lead Physician reports to you
• Home base in Auburn; leadership meetings rotate between Auburn and Grass Valley on a set schedule, and a GSA vehicle is available
• Year one is about relationships and judgment: learn the medical team and the other departments, evaluate, then bring proposals to the CCO. Chapa-De is deliberately light on red tape and is not looking for a bull in a china shop.

Compensation and package

• $340,000 to $380,000 base salary. The top of the range is reserved for physicians with direct leadership experience in another FQHC.
• Signing bonus available with a two-year commitment, and relocation assistance sized to your move. Both are set with the candidate rather than quoted in a posting.
• Annual increases and market adjustments
• Employer-paid medical (Blue Shield PPO or Kaiser HMO), dental and vision for the employee, with 80% toward dependent medical coverage
• Up to 27 vacation days, 11 paid holidays, and accrued sick time
• Retirement match up to 6%, CME allowance of up to $2,500 and 5 days, license renewal assistance, and a DEA application fee waiver under the Indian Health Care Improvement Act
• Malpractice and tail coverage through FTCA, so claims run against the federal government

What Chapa-De is looking for

• MD or DO, board certified in Family Medicine; board-eligible physicians actively pursuing certification will be considered. The seat oversees a family medicine practice and sees patients of all ages, so Family Medicine specifically.
• Active California license, or the ability to hold one before a spring 2027 start. Chapa-De supports expedited licensure for its Auburn site.
• Leadership experience in a primary care setting, in any form: lead physician, site or clinic director, associate or full medical director, chief of staff, department chief. What matters is evidence that you led providers, not the title.
• Two or more years of direct primary care delivery
• FQHC, IHS, or tribal health experience is highly preferred. If your background is a large health system, show the adaptability that closes the gap.
• Authorized to work in the United States without sponsorship, now or in the future. Chapa-De does not sponsor visas.
• Current BLS, DEA registration, and a valid driver's license

How the process works

• Two interview rounds of 60 to 90 minutes each with the executive team, including the Chief Clinical Officer. Round one can be virtual; round two is on site with a tour of the clinics.
• Target start is spring 2027, which allows for credentialing (about three months) and a thoughtful handoff.
• Preference in filling vacancies is given to qualified Indian applicants in accordance with the Indian Preference Act.
