Location:On-site in Brooklyn, NY 11218
Key Responsibilities
Virtual Visit Facilitation
Initiate outbound calls to patients at scheduled appointment times to prepare and connect them to their provider (video or telephone visits).
Ensure patients are ready for the visit, including confirmation of availability, technology readiness, and required documentation.
Coordinate real-time handoff between patient and provider to support on-time visit start.
Appointment Reminders & Outreach
Conduct appointment reminder calls, same-day confirmations, and follow-up outreach to reduce no-shows.
Communicate schedule changes, delays, or cancellations to patients and providers as needed.
Document all outreach attempts and patient interactions according to organizational standards.
Provider Schedule Support
Monitor provider schedules throughout the day and proactively address gaps, late arrivals, or connectivity issues.
Serve as the first point of contact for visit-related operational issues during clinic hours.
Documentation & Systems
Accurately document visit status, outreach attempts, and outcomes in the designated systems (e.g., EHR, scheduling platform, call system).
Maintain up-to-date appointment notes and flags related to visit readiness or barriers.
Adhere to HIPAA and organizational confidentiality policies at all times.
Coordination & Communication
Collaborate closely with providers, medical assistants, nurses, and front office staff to ensure smooth visit flow.
Provide clear, professional communication to patients regarding visit expectations and next steps.
Support process improvements by identifying recurring visit barriers or scheduling issues.
Required Qualifications:
High School Diploma or GED.
Bilingual Fluency in Cantonese.
1-2 years of telehealth experience.
Proficiency in using EMR systems, experience in AdvancedMD a plus.
Strong grasp of patient care procedures and medical terminology.
Excellent interpersonal skills to effectively interact with patients and healthcare professionals.
Strong organizational skills to manage patient records and appointment scheduling.
