Job Description - Medical Records Clerk

JOB SUMMARY:

Under the direction of the Medical Records Manager, the Medical Records Clerk receives, reviews, prepares, tracks and releases medical records in response to medical records requests in compliance with Agency policy and all applicable regulatory requirements.

ESSENTIAL DUTIES:
  • Process medical record requests in accordance with Agency policy, including, but not limited to, reviewing the request, ensuring it is valid and complete, correcting errors when necessary, preparing the record for release, and releasing in compliance with Agency requirements.
  • Track the release of all medical records through the Maintenance of the Agency Release of Information data base.
  • Prepare applicable files for storage and sends them to off-site storage.
  • Retrieve/return files from storage as necessary and track requested records per Agency policy.
  • Attend scheduled staff meetings, supervision, and on-going training.
  • Assist Affiliate medical records teams as directed by the Medical Records Manager. 

OTHER DUTIES:
  • All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  • Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment.  All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  • Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  • Other duties as assigned by Management.

QUALIFICATIONS:
  • Education: A High School Diploma required. Bachelor’s degree preferred.
  • Skills/Competencies:

    • Professional & Ethical Conduct: Adhere to professional values such as honesty, personal responsibility, and accountability; Applies ethical concepts within scope of work and adheres to Agency policies and procedures.
    • Collaboration and Teamwork: Function effectively as a member of a professional team that includes employees, clients and family members.
    • Problem Solving & Decision Making: Recognize problems and responds appropriately; gathers information and sorts through it to identify and address root cause issues; makes timely decisions.
    • Service Orientation: Anticipate, recognize, and meet the needs of others, whether they are clients or not.
    • Technical Proficiency: Demonstrate competence in utilizing Agency computer systems and software as required to perform essential job functions.
  • Experience: One to three years related work experience required.
