Summary:
Under the supervision of a pharmacist, the medication history technician (MHT) will obtain medication histories through interviews with patient/family/care partner. The MHT will use available technology to pull in external full history from patients pharmacies.
Responsibilities:
Obtains and clarifies patients home medication list including prescription, OTC, and herbal products. Any bottles brought in will be promptly returned to patients family/care partner after all important information has been transcribed.
Responds to nurses, physicians, pharmacists, or other requested to appropriately manage, triage, and ensure follow-up in a timely manner regarding med history information.
Obtains patients allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR) including symptoms, reactions, and severity.
Directs, triages, and communicates to appropriate caregiver all questions and inquiries from patient/family/care partner that are outside the scope of the MHT.
Identifies and participates in ongoing quality assurance and improvement process to ensure an appropriate med history list is obtained.
Fulfills all compliance responsibilities related to the position.
Performs other duties as assigned.
Education Skills Experience:
Required: High School Diploma or equivalent.
Minimum Experience: 1 year
PTCB Certification within in 1 year
License: Registered Pharmacy Technician
Within in 30 days with Connecticut Department of Consumer Protection for those working in CT. Within in 6 months with Board of Pharmacy for those working in NY.
Desired: Retail pharmacy Technician experience. Computer skills, ability to multi-task and work independently.
Working Conditions:
Manual: significant manual skills/motor coord & finger dexterity
Occupational: Little or no potential for occupational risk
Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force
Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
Salary Range: $18.13 - $34.50 Hourly
