Medication Specialist

Pay Range: $24.88 - $28.61/hr

Schedule: Monday 8am - 4:30pm

The Medication Specialist will provide oversight, accountability, administrative and clerical support to Outreach programs in the office and virtually. They will assist CST TLs in capturing all medication change opportunities and follow through on the medication change process. They will bring some level of medication knowledge and help educate the CST teams on the importance of safe medication practices. They will help manage systems of communication and documentation between the clinical staff, tracking, and administrative needs which ensure compliance while maintaining a professional and efficient relationship with internal and external stakeholders.

Essential Responsibilities

• Provide specialized Medication administrative and clerical support for CST and RS levels of care; i.e. tracking appointments; following up on medication changes; researching in Electronic Health Records and auditing physical MAR and med boxes to ensure compliance; documentation reminders, collaborating with TLs and teams about medication issues, assisting with audits with Trilogy EHR and clinic EHR; completing CIRs as needed


• Provide support to Team Leaders and Program Managers with team operations and tracking to support safe medication storage and reduce opportunities for error; i.e. regular attendance at team meetings, maintenance of medication lists, completion of tracking forms and logs, etc.


• Act as a liaison between the clinic, pharmacy and CST teams to ensure medications are being followed up on in a timely manner within the CST level of care.


• Ability to quickly grasp systems and build relationships with multiple types and levels of staff


• Ability to understand the medication policy at Trilogy and support program outcomes by developing workflows to complete tracking in a timely and accurate manner


• Ability to navigate and enter data in various systems, Excel, Microsoft Forms, etc.


• Assist in retrieving and organizing updates and needed follow-up in daily team meetings;


• Assist with medication reconciliation, maintenance of medication sheets, and update systems accordingly when medication changes are received. Assist with the controlled substance process, which includes sign-in/sign-out of medications per policy.


• Attend weekly CIR meeting about medication errors observed throughout the agency; provide administrative and collaborative support to the Medication Error Reduction Taskforce.


• Provide on-site support to employees including training on medication interactions, high risk medications with substance use and other relevant topics to the Outreach work


• Ensure client documentation is received and scanned into appropriate systems.


• Actively participate in daily group chats with assigned teams to support in navigating questions regarding medication changes, medications, etc.


• Support teams in tracking client medical appointments and ensuring that clients are following up with providers i.e. psychiatry, primary care, and specialists


• Assist with recognition announcements and planning of employee appreciation activities


• Represent Trilogy in a positive and professional manner; and communicate effectively and appropriately to others about potential organizational issues.


• Support with communication to funding bodies, Strategy team, RNs, etc related to client outcomes and goals achieved with greater medication independence


• Perform other related duties and/or projects and participate in committees/workgroups as assigned

Qualifications

• Thorough knowledge of psychiatric and common medical medication terms, contradictions, side effects, etc through a demonstrated work experience as a pharmacy tech, in a clinic setting, or 2+ years of performing outreach medication monitoring.


• Thorough knowledge of Microsoft applications (Outlook, Teams, Word, Excel, etc.), report generation, and database management tools


• Demonstrated work experience with medication compliance, processes, policy, safe storage and risk reduction


• Excellent customer service and organization skills


• Provide thorough communication to all teams, departments, and Supervisors


• Ability to be flexible and adaptable in a variety of situations
