Mid-Level Provider (Family Nurse Practitioner or Physician Assistant)

Location: Peterstown Health Center (Primary Location)

Monroe Health Center is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team. As a Federally Qualified Health Center (FQHC), we are committed to providing high-quality, patient-centered primary care to individuals and families of all ages, regardless of their ability to pay.

The selected provider's primary practice location will be Peterstown Health Center. This position will also provide Provider coverage at other MHC Health Center locations as needed.

Responsibilities

• Provide comprehensive primary care services to patients of all age groups.


• Diagnose and treat acute and chronic medical conditions.


• Perform preventive health screenings and patient education.


• Collaborate with physicians and other healthcare professionals to coordinate patient care.


• Maintain accurate and timely medical documentation in the electronic health record.


• Participate in an on-call rotation shared among providers.

Qualifications

• Current licensure as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) in West Virginia (or eligible for licensure).


• Current DEA registration (or ability to obtain).


• Preferred: Minimum of two (2) years of family practice or primary care experience.


• Commitment to providing compassionate, high-quality care to diverse patient populations.

Schedule

• Full-time position


• No weekend clinic hours


• Shared on-call rotation
