Minimum Data Set Nurse LPN SNF

Lexington Country Place is seeking a dependable and detail-oriented MDS LPN to join our nursing team.

Shift Details:

8 hours per week

The MDS LPN is responsible for coordinating and completing MDS assessments, supporting accurate clinical documentation, and working with the interdisciplinary team to ensure quality resident care and regulatory compliance. 

 Responsibilities 

• Complete and coordinate MDS assessments accurately and within required deadlines.
• Review clinical records and documentation to support accurate MDS coding.
• Collaborate with nursing, therapy, dietary, social services, and other departments.
• Participate in care planning and interdisciplinary team meetings.
• Monitor resident changes and ensure appropriate documentation.
• Assist with Medicare/Medicaid documentation and regulatory requirements.
• Maintain compliance with applicable CMS, state, and facility requirements.

Qualifications

• Current LPN/LVN license in the state of employment.
• Previous long-term-care or skilled-nursing experience preferred.
• MDS experience preferred, but training may be available for the right candidate.
• Strong organizational, communication, and documentation skills.
• Detail-oriented with the ability to meet deadlines.
• Ability to work effectively as part of an interdisciplinary team.
