Navigator RN Full-time Variable

Under the direction of the Director or Manager of Nursing, the Wound Care Navigator coordinates patient care, implements wound care policy, ensures wound assessment and prevention standards, and supports program outreach for patients with wounds and at risk for skin breakdown. The Wound Care Navigator monitors patients from admission through discharge, assisting them with navigation through the healthcare system, providing education and emotional support, and collaborating with interdisciplinary teams to develop programs that enhance wound care and prevention.

Required Qualifications:

• Bachelor's Degree in Nursing

Preferred qualifications: 

• Master's Degree in Nursing


• 1 year of wound Care Experience.
